Altar Set Up: Friday, November 1st, from 3PM until Saturday, November 2nd at 10AM.
Altar Tear Down: must be complete by Sunday, November 3rd, 2013 at 7AM.
Please submit the above form, then mail the attached waiver (link given on following confirmation page) along with a $50 Refundable Deposit (you will not receive the refund if you do not show up) to:
CANCELLATION: To receive a refund, cancellation must be made no later than October 19th, 2012.
STEP 2: After all paperwork is submitted, you will receive a map indicating your parking and Altar setup space location.
