12th Annual Celebration

12PM - 12AM

Altar Set Up: Friday, October 21, 2011 • 5PM - 12AM & Saturday, October 22, 6AM - 11AM.
Altar Tear Down: Sunday, October 23, 2011 by 2AM.













Please submit the above form, then mail the attached waiver (link given on following confirmation page) along with a $50 Refundable Deposit (you will not receive the refund if you do not show up) to:

CANCELLATION: To receive a refund, cancellation must be made no later than October 15th, 2011.

STEP 2: After all paperwork is submitted, you will receive a map indicating your parking and Altar setup space location.
