Altar Set Up: Friday, October 28th, from 3PM until Saturday, October 29th, 10AM.
Altar Tear Down: must be complete by Sunday, October 30th, 2016 at 7AM.
Please submit the above form, then mail the attached waiver (link given on following confirmation page) along with a $50 Refundable Deposit (you will not receive the refund if you do not show up) to:

CANCELLATION: To receive a refund,
cancellation must be made no later than
October 21st, 2016.
Please make check payable to Hollywood Forever, Inc. Participation is not confirmed until check has been received.
STEP 2: After all paperwork is submitted, you will receive a map indicating your parking and Altar setup space location.
